Digital registration or a form
Two adults can create the proxy in Läkemedelskollen using e-identification.
They can instead sign a form and submit it at a pharmacy, where identity is checked.

Consent and validity
Both people must consent to the processing of their personal data before the proxy works.
The maximum duration is four years, after which a new proxy must be created rather than extending the old one.
